Fans were able vote for one of three cover designs featuring Bautista via a Facebook vote.
This marks the second year Canada has had its own edition of the game.
Pittsburgh Pirates centre-fielder Andrew McCutcheon won a fan vote to be the U.S. cover athlete.
The game is out March 5 for the PlayStation 3 and PS Vita.
